Prices include one custom design, in one standard color, imprint on sole/rear cuff of mid-calf length socks, sole of ankle-length socks, or one side of OB/GYN Stirrup Covers. Pricing is subject to change without prior notification.

FREIGHT TERMS: Prices do not include shipping charges. All products are shipped F.O.B. Ohio. Split shipments will incur a $20.00 handling fee per additional location. Special packaging/shipping requirements will incur additional fees.
ARTWORK: Camera-ready artwork is any professionally prepared black & white rendering of your logo, emblem, line art or copy. Example: business card, letterhead. Photocopies (Xeroxes) or fax transmissions are not acceptable. Acceptable formats are Photoshop 7 or Freehand 10 versions or below. Artwork to be coordinated through distributor. If no camera-ready artwork is available, manufacturer’s graphic department will prepare your artwork for an additional fee of $30.00/x per item design.
PAPER PROOFS: There is no charge for the first 2 black & white paper proofs for new orders. Additional paper proofs are $10.00/x each proof. Not responsible for any errors on approved paper proofs.
PROTOTYPE SAMPLE: Upon approval of paper proof, one prototype sample (per design) will be available at no charge. Revisions or changes will incur $50.00/x per change.
RETURN OF ARTWORK: Artwork will be returned only if noted on purchase order.
METHOD OF IMPRINTING: Silkscreen process using plastisol puff ink. Heat curing process forms a skid-resistant tread.
OVERRUNS AND UNDERRUNS: Orders are considered complete with a 5% overrun or underrun. To guarantee exact quantity, add 5% to product cost. Must be noted on purchase order.
PURCHASE ORDERS: All purchase orders must be submitted in writing. Packing lists are not provided. If packing list must accompany order, please remit 72 hours before scheduled ship date. Custom labels must be provided.
CHANGES &CANCELLATIONS: Subject to all costs incurred.
TERMS: 50% with order. Balance due at time of shipment unless credit has been established.
RETURN INFORMATION: No returns on customized goods.
PRODUCT GUARANTEE: The products are guaranteed against defects in material or workmanship. However, we reserve the right to have defective merchandise returned or inspected in the field before a credit may be issued.
COPYRIGHT, TRADEMARKS AND LICENSING: We have no responsibility for determining who holds title to a design, trademark or license. All liabilities of this nature remain with the customer.
